Opioid Health Home is a MaineCare-funded, whole person treatment service for individuals diagnosed with an opioid use disorder and at risk for chronic health problems.

OHH is an innovative treatment approach that helps people seeking to recover from opioid misuse and their associated mental and physical problems get better and live a healthy, productive life. Work with a care team to develop a personal plan of care for you. Your team includes:

• Medication assisted treatment provider

• Substance use counselor

• Peer Recovery Coach

• Nurse Care Manager

• Patient Navigator
